You are now scheduled for a telemedicine intake evaluation with Emerge Ketamine!

This evaluation will use a HIPAA compliant telemedicine platform. An Emerge Ketamine Physician will review your medical history with you, evaluate your candidacy for ketamine therapy, and give you information about every step of the process for receiving a ketamine infusion in our suite or in your home.

The appointment: At the date and time of this appointment, you will receive a text message or email (based on your preference) with a link to open the video visit. Please be sure to have your device charged and notifications enabled so that you do not miss this message.

Required forms: You’ll also receive an email from us shortly containing an intake questionnaire and a communications consent form. The Osmind medical intake questionnaire and the Consent for Telehealth and Electronic Communication must be completed prior to this visit. If these forms are not completed, this visit will be rescheduled or canceled.
